Make an Appointment

Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (615) 342-6800 during regular business hours or utilize our online appointment system. Please call in advance to schedule routine office visits. We make all efforts to accommodate same day appointment requests. We encourage all patients to schedule follow-up visits as you leave the office. We strive to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.

New Patients

New patients are asked to arrive 30 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, driver license or other form of ID, a list of all prescription and over the counter medications you are currently taking, copies of any recent test result or lab results, and your insurance card to your appointment.


We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.

You can also use our secure Online Bill Pay option to pay current statement balances online without setting up an account.

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

We recommend that you call your insurance company to verify that we participate with your plan.

Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments.

Refill Requests

We are happy to assist with most medication refill requests. There are several ways to request a refill request. You may contact the office during regular office hours, please feel free to contact us at (615) 342-6800 to provide the specifics on your medication refill request. You may also access the patient portal to submit a request electronically. Your pharmacy can submit a refill request electronically or by fax.

After-Hours & Emergencies

If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call (615) 342-6800 and our answering service will assist you and contact the provider on call.
